How do I tell my primary care provider to refer me to Modern Pain Consultants?
Discuss your pain with your primary care doctor. Your doctor will then provide you with a referral order. In some rare cases, a referral is not needed.
What should I bring to my first appointment?
For your convenience, you can now download and complete your NEW PATIENT FORMS on our website. Once the forms are completed you can send the forms via email, fax (708-482-4502), standard mail, or bring the completed forms to your first appointment. If you have had any recent imaging or consultations with other specialists, please fax reports to 708-482-4502.
